FAITH & DIABETES WORKSHOP

Diabetes Self-Management Workshop Series will teach those with pre-diabetes or diabetes wisdom to manage this journey. Attendees are caretakers of someone with diabetes are merely interested in learning more about prevention of diabetes practical strategies to manage the condition through learning a lifestyle modification. We are in partnership with Institute of Health & Spirituality & Cities Changing Diabetes. – Cost: FREE – Registration: Required

The pre-survey collects more detailed information than the enrollment form. This survey should be completed as a part of the first DSMES class in a series. It is expected to take 10-15 minutes to complete.

The post-survey collects information in parallel to the pre-survey, which will allow us to see changes over time, as well as the “stickiness” of those changes and knowledge gained.
